Patients who received their COVID vaccine at Primary Health may now request a digital vaccine record, which may be helpful when traveling or attending events.
How to access your digital COVID vaccine record:
- Send us an email with the patient name and date of birth (This method is not secure. For secure communication, you may request your record through your Patient Portal.)
- Our staff will send a QR code to your Patient Portal by the following business day
- Your QR code will be available under Medical Records>Documents. You must use a desktop computer to access this document.
- Scan the QR code with your phone's camera to save your vaccine record to your phone*
You may also request a hard copy of your QR code in the clinic at the time you receive your vaccine.
*Your vaccine record can be saved to any app that uses the SMART Health technology, including Apple Wallet and CommonPass. Learn more about SMART Health Cards. If you need your digital record for travel, please be sure to use the app required by the city, state or country you are visiting.
